The purpose of this form is to nominate persons (men and women) to be considered for the church’s lead teams (Servant Lead Team, Finance Ministry Team, Personnel Ministry Team, Missions Team, etc.). The primary consideration is spiritual maturity as characterized in 1 Timothy 3:1-13 and Titus 1:5-15, however some team members should have aptitude for their respective areas (financial, human resources, etc.).
